Senior doctor reading through a patient folder held by a nurse
Why a programme

The danger lies in the years between check-ups

Most strokes, kidney failures and heart attacks linked to hypertension and diabetes do not come out of nowhere. They follow years of readings that were a little too high and not looked at closely enough.

The programme closes those gaps. Members are seen every month, their numbers are kept on a single chart, and nothing is left to memory: not the yearly eye check, not the kidney test, not the medicine review.

  • For adults with hypertension, diabetes or both
  • Especially helpful after a hospital admission or a stroke scare
  • Suitable for elderly patients, with a home option
  • A relative can be kept informed with your consent
How it works

Four steps, repeated faithfully

  1. 1

    Enrolment review

A 45-minute first assessment with a doctor: your history, your medicines, full blood tests, an ECG and kidney tests to set your baseline.

  2. 2

    A written plan

Personal targets for blood pressure and sugar, a medicine schedule you understand, and dietary changes that suit your household.

  3. 3

    Monthly reviews

Readings checked and charted each month at the practice or at home, with medicines adjusted only when the trend calls for it.

  4. 4

    The yearly full check

Once a year we repeat the complete kidney, heart, eye and foot checks and sit down with you to review the year.

Questions

About the programme

Who is the programme for?

Adults living with high blood pressure, type 2 diabetes, or both. Patients with heart or kidney complications may join too, alongside their specialist care.

Are medicines included in the monthly fee?

No. Medicines are charged separately, so you pay only for what you take. Our pharmacist will always tell you honestly about cheaper equivalent options.

Can I change plans?

Yes, from the following month. Many members move to Close Watch after a change in medicines, and back to Steady once things settle.

What happens if I miss a monthly review?

The nurse telephones you or your nominated relative within two days to rebook. Missed reviews are the most common reason readings drift, so we take them seriously.

Will the programme cure my hypertension or diabetes?

These are long-term conditions that are managed rather than cured. The aim is steady control, fewer complications and a fuller life for as long as possible.

Enrol yourself or a parent

Begin with an enrolment review. We will explain each plan in person and recommend the one that fits, with no obligation to join.
